How do I make an initial appointment?

We only schedule initial appointments after receiving the preliminary New Patient Forms.

Step 1: Complete and return the New Patient Forms, either by FAX or email. When they are received you will be billed, via email, for a holding deposit of $100 (deductible from total initial consult cost). 

Step 2: Following payment of deposit we will schedule your initial appointment. Meanwhile you can gather relevant and submit medical records and other items which should arrive 48 hours before your consultation.

The New Patient Checklist, available below, can assist you through the registration process. It includes a list of the needed medical records.
